Champlain Valley Family Center offers Care Management as part of AHI's Health Home.
A Health Home is a care management service that connects community and social supports with health care, and organization of medical and behavioral health care. It is a partnership of primary care providers, hospitals, behavioral health organizations and community-based organizations.
Care Managers provide a single contact for healthcare and community service needs for a specific population (NYS Medicaid or Medicaid eligible recipients who meet certain criteria, such as having two chronic medical conditions) in order to improve care coordination. It is the hope that all of the caregivers involved with an individual communicate with each other with an eye toward a better focus on their mutual client's varied needs.
